You can change your Facebook page URL without having submitting any request or work directly with a Facebook representative.

Previously, pages with more than 100 Likes could not change their URL. To change a page URL you needed to fill out a form, and there were no guarantee of changing in the URL because request could be rejected. Now, Facebook has removed that contact form from Facebook site and given page owners the option to change their URL a single time. To change the URL of your page follow the given instructions: Edit Page –> Update Info –> Basic Information –> Change Username. 

Upon doing so, users are reminded that they can only change their username once. From this page, users can check the availability of their desired vanity URL. This is similar to what Facebook has allowed for user profiles since 2009. 

But page owners, still cannot change the complete name of their page if they have more than 200 Likes.

After adding above code you will find this tag </head> in your Template >> HTML >> Proceed and right before it paste below code:

Customization:

There are two options to customize this widget; the first one is changing bloggers-tips with your Facebook page user name

The second is float:right. However, if you want this widget to appear at left side then change the property right to left in float:right;.
